Mechanism of action

Raw tadalafil powder is a selective phosphodiesterase 5 (PDE5) inhibitor that can produce a variety of downstream effects. The most common therapeutic effect is smooth muscle relaxation. Patients may experience ED for a variety of reasons, including cardiogenic, neurological, vascular, iatrogenic, or endocrine. These reasons cause penile smooth muscle relaxation dysfunction by destroying neuronal signals or directly affecting smooth muscle cells. During sexual arousal, non-adrenergic non-cholinergic (NANC) neurons release nitric oxide (NO). Nitric oxide stimulates guanylate cyclase, which converts guanosine triphosphate into cyclic guanosine phosphate (cGMP). 2,3 cGMP activates cGMP-dependent kinase (PKG) in the signal cascade, activates K+ channels, causes Ca2+ channel inhibition, inhibits platelet activation, and inhibits smooth muscle cell proliferation, and induces apoptosis at the same time. This signal cascade is attenuated by PDE5, which destroys the phosphodiester bond of cGMP and converts it to GMP. Inhibition of PDE5 by tadalafil increases signaling through the PKG cascade, which supports the relaxation of penile smooth muscle during sexual arousal by reducing the entry of Ca2+ into smooth muscle cells. This relaxation of smooth muscles fills the corpus cavernosa with blood, creating an erection.

In PAH, pulmonary blood pressure rises due to multiple mechanisms caused by endothelial dysfunction. Decreased production of NO and prostacyclin will reduce vasodilation signals, while excessive production of endothelin-1 and thromboxane will increase vasoconstriction. Inflammation, thrombosis, and hypoxia can then cause blood vessels to remodel, further reducing the size of the lumen. The resulting increase in blood pressure reduces the gas exchange capacity and increases the afterload of the right ventricle, producing symptoms of dyspnea, fatigue and dizziness, and leading to right-sided heart failure. Tadalafil promotes smooth muscle relaxation by enhancing NO-cGMP signaling, thereby exerting its therapeutic effect in PAH, just like ED.

Finally, tadalafil is used to treat BPH. BPH causes urinary dysfunction through excessive proliferation of the prostate epithelium and smooth muscle layer. The increase in the size of the prostate will hinder the passage of urine through the urethra, and the residual volume will increase due to incomplete emptying. Tadalafil does not seem to exert its benefits through the relaxation of prostate smooth muscle. On the contrary, it may exert its effect by increasing the combination of oxygenation and reducing inflammation, thereby reducing tissue remodeling, and inhibiting cell proliferation through the cGMP cascade.

Compared with other PDE5 inhibitors, the reduced affinity for PDE6 can explain the reduced incidence of visual side effects because PDE6 is present in the eyes and contributes to color perception.

Dosage

The recommended dose of this product is 10mg, which is taken before sexual activity and is not affected by food. If the effect of taking 10mg is not significant, you can take 20mg, at least 30 minutes before sex.

Interdiction

Clinical studies have shown that tadalafil can enhance the antihypertensive effect of nitrates. This is thought to be the result of the combined action of nitrates and tadalafil on the nitric oxide/cGMP pathway. Therefore, it is contraindicated in patients taking any form of nitrates.

Sexual activity poses potential heart risks to people with heart disease. For patients with pre-existing heart disease, physicians should consider the potential cardiac risks of sex. The following clinical trials have not included patients with the following cardiovascular diseases, so tadalafil is strictly contraindicated in these groups:

Precautions

Before considering drug therapy, a medical history and physical examination should be taken to diagnose male erectile dysfunction and to identify possible unknown etiologies.

Because the risk of cardiovascular disease is somewhat related to sexual behavior, physicians should consider the cardiovascular health of men with erectile dysfunction, including tadalafil. The vasodilatory properties of tadalafil cause a mild, transient decrease in blood pressure, which may enhance the hypotensive effect of nitrates.

Serious cardiovascular disease, including myocardial infarction, unstable angina, ventricular arrhythmia, shock, transient ischemic attack, etc., have been observed in clinical trials of tadalafil. In addition, hypertension and hypotension (including orthostatic hypotension) are occasionally seen in clinical trials. Most of the above-mentioned patients have pre-existing cardiovascular disease factors. However, it is currently uncertain whether these events are related to these risk factors.

There is limited information on the clinical safety of this product in patients with severe hepatic impairment (Child-Pugh class C); if prescribing in such patients, careful benefit/risk assessment of each patient is required by the prescribing physician.

No priapism has been reported in clinical trials of tadalafil. However, priapism has been reported with another PDE5 inhibitor, sildenafil, and patients should be advised to seek immediate medical attention for erections that last 4 hours or more. If priapism is not treated promptly, it can lead to the destruction of penile tissue and permanent loss of erection.

Tadalafil should be used with caution in patients who are prone to priapism (eg, sickle cell anemia, multiple myeloma, or leukemia), or those with abnormal penile anatomy (eg, angulation, malformed penis, cavernous fibrosis) or Peyronie's disease).

Diagnosis of erectile dysfunction should include identification of the underlying etiology and appropriate treatment followed by appropriate evaluation. It is unclear whether this product is effective in patients with spinal cord tract injury, patients who have had pelvic surgery, and patients who have undergone prostatectomy.

Tadalafil should not be used in the following patients: patients with hereditary galactose intolerance, or patients with galactolytic enzyme deficiency, or patients with glucose-galactose malabsorption.

In patients who are taking alpha-blockers, such as doxazosin, concomitant use of this product may result in symptomatic hypotension in some patients. Therefore, concomitant use of tadalafil with alpha-blockers is not recommended.

Special care should be taken if tadalafil is prescribed to patients who are taking potent CYP3A4 inhibitors (eg, ritonavir, saquinavir, ketoconazole, itraconazole, erythromycin). Because tadalafil has been found to enhance the exposure (AUC) of tadalafil in combination with this class of drugs.
The safety and efficacy of this product in combination with other erectile dysfunction treatments have not been studied. Therefore, such combination is not recommended.

In a dog trial, tadalafil was given at doses of 25 mg/kg/day or more per day for 6-12 months (at least 3 times higher than the single dose of 20 mg/kg/day in humans) exposure rate [range 3.7-18.6]), some dogs were found to have degeneration of the vas deferens epithelium, which led to a decrease in sperm count, and the results of a 6-month study in healthy volunteers in two groups showed that in humans The clinical phenomena observed in the dog trials described above did not occur. The effects of long-term continuous daily administration of tadalafil have not been established, so continuous daily administration is contraindicated.

Tadalafil had no or negligible effect on the ability to drive and operate machinery. Specific studies on this potential effect have not been conducted. Although the frequency of vertigo reported in the placebo and tadalafil groups in clinical trials was similar, patients should know their response to tadalafil before driving and operating machinery
